Filing a Claim

Contact Information: Your address, telephone number, fax number, and email address for contact purposes
Good Faith Statement: A statement affirming that you believe the use of the material is not authorized by the copyright owner, its agent, or the law
Accuracy and Authority Statement: A declaration that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act on behalf of the copyright owner
